As in Diablo II, beating Baal opens the next difficulty. A hero who beats Baal can continue into Nightmare, then Hell, at her level with her build and gear, or a fresh run can start on a difficulty you have opened.

Champions
Champion packs come in five kinds, each as likely: Champion, Ghostly (resists 80% of physical damage), Fanatic, Possessed and Berserker (a quarter of a champion's life, but hits harder). Their chests give level-ups through Act II, then stat points or +1 to a skill tree.
After Hell
Beat Baal on Hell and the Terror tiers open: one act of Hell at a time, from monster level 86, with affixes that pay in Magic Find, experience and gold.
